What is the Imaginal Exposure Practice Log?

Imaginal exposure involves repeatedly revisiting the trauma memory to process it emotionally and reduce avoidance. Tracking practice supports engagement and shows progress.

Why it works

Imaginal exposure is a core component of prolonged exposure therapy (Foa et al., 2007). Trauma-focused therapies with a memory processing component are strongly recommended (ISTSS, 2019).

When to use it

  • Prolonged exposure therapy
  • Between-session listening practice
  • Monitoring habituation and learning
  • Supervision

How to use it in session

  1. Record in session.
  2. Listen daily as agreed.
  3. Rate distress.
  4. Note thoughts.
  5. Review together.

Clinician tips

  • Use only within a structured treatment.
  • Check for over-engagement or dissociation.
  • Pair with grounding afterwards.

Worked example

Day 1: 70 / 90 / 60. Day 7: 40 / 55 / 25. Changed: the memory feels more like the past.

Frequently asked questions

Do I have to do this alone?

Imaginal exposure is planned with your therapist; home practice follows what you agree in session.

What if I feel worse at first?

Distress often rises early on. Tell your therapist how it’s going.
