Key facts
What is the Coherent Breathing (5.5 Breaths per Minute)?
Coherent or resonance breathing involves slow, even breathing at roughly five to six breaths per minute, without breath holds. This rate is associated with maximal heart rate variability for many adults and is the basis of HRV biofeedback training.
Why it works
Breathing near resonance frequency increases heart rate variability and baroreflex activity, and HRV biofeedback has shown benefits for anxiety, stress and depressive symptoms (Lehrer & Gevirtz, 2014). Systematic reviews link slow breathing with greater parasympathetic activity and calm (Zaccaro et al., 2018).
When to use it
- Generalized anxiety and stress
- Clients who dislike breath holds
- Daily regulation practice
- Alongside HRV biofeedback
How to use it in session
- Demonstrate the 5.5-second rhythm with the timeline.
- Practise together for 2-3 minutes.
- Offer apps or audio pacers for home.
- Build to daily 10-minute practice.
- Review calm ratings and adjust.
Clinician tips
- If 5.5 seconds feels long, start at 4 and gradually slow down.
- Avoid overbreathing: the breath should be gentle, not deep and forceful.
- Practise when calm so it’s available when stressed.

Frequently asked questions
Is coherent breathing the same as box breathing?
No. Coherent breathing has no holds and a slightly slower, continuous rhythm. Box breathing uses equal in-hold-out-hold phases.
Why 5.5 breaths per minute?
Research suggests most adults’ resonance frequency falls around 4.5-6.5 breaths per minute; 5.5 is a common midpoint.
