What is Loneliness in Later Life Plan?

When to use it

  • Older adults adjusting to change
  • Loneliness and life review
  • Families and caregivers supporting memory changes

How to use it in session

  1. Explain the purpose and check that the client wants to do this.
  2. Complete the table: person, group or place, how often i would like contact, first step.
  3. What gets in the way (transport, hearing, health, confidence).
  4. What would help with that.
  5. Agree one small next step.

Clinician tips

  • Adapt pace, print size and session length as needed.
  • Respect autonomy and include the person in decisions.
  • Attend to caregiver wellbeing as well as the person with memory changes.
