Key facts
What is the Body Neutrality & Appreciation Reflection?
Body neutrality and body functionality approaches shift attention from appearance to what the body can do and how it feels. They offer a middle path for clients who find ‘body positivity’ unrealistic, and align with research on positive body image, which emphasizes appreciation, acceptance and respectful care (Tylka & Wood-Barcalow, 2015).
Why it works
Positive body image is distinct from the absence of negative body image and is associated with better wellbeing and self-care. Integrating attunement and mindful self-care into eating disorder treatment supports recovery (Cook-Cottone, 2015).
When to use it
- Body dissatisfaction and eating disorders
- Chronic illness, disability or body changes
- Pregnancy and postpartum body changes
- Teens and adults affected by social media comparison
How to use it in session
- Introduce neutrality as respect, not forced love.
- Complete the functionality cards together.
- Practise reframing two or three common harsh thoughts.
- Choose one caring action for the week.
- Reflect on what changed in session.
Clinician tips
- Respect cultural, gender and disability contexts in body talk.
- Watch for body checking and avoidance as maintaining behaviours.
- Go gently; this work can stir grief.

Frequently asked questions
Is body neutrality the same as body positivity?
No. Body positivity encourages loving your body; body neutrality focuses on respect and function without requiring positive feelings about appearance.
Can this be used with men and non-binary clients?
Yes. Body image concerns affect people of all genders, and the exercise can be adapted to any body.
